From my experience, pretty much yes!
My guide tends to be very courteous about it though, like she's waiting for an invitation. She will wait until I vocalise something before commenting, but sometimes if I fumble for words, she knows exactly what I mean to say anyway.

i can hear my own breathing during physical exercise and songtechnique-training. its closer than anything ive ever heard and the voice becomes very loud, but its only my own breathing taking a way through my head or something, not sure this is the same thing youre experiencing. hmm... yepp, thats it.
if you hear your voice loudly in your head when you talk as i do, youd better think again, then its nothing unphysical.
