I've studied this topic myself for many years and it seems as though there are probably more unwanted/unplanned releases of Kundalini than one might suspect. Physical injuries or illnesses seem to be the source most likely to release the Kundalini, along with unwise spiritual experimentation.
I've read quotes from clairvoyant investigators which say that about half the populations in mental facilities suffer from premature/unwanted/uncontrollable Kundalini forces! Another source of problems is unbalanced body chemistry which interfers with the connection and normal functioning between the physical brain and the higher spiritual bodies. One way this manifests is "throwing open the doors" of the spiritual sense faculties which are meant to be shut for the untrained and undisciplined average mind. They become lost in an ocean of sensory data which they cannot cope with.
