Clapping your hands or rubbing them together can work to extend your stay.  I've read (haven't tried this one personally) that you can tense the muscles in your abdomen.  If you strain the muscles too much you run the risk of waking up back in the physical, though.

If whatever you try doesn't work and you land back in the physical, you can usually always go right back out again.  Just don't move your physical body and 'think and feel' yourself right back where you were. 

I usually go out 2 or 3 times in a row.  My husband goes out like 5 times in a row.  He's a machine! lol  Just keep in mind that if you want to be sure that you remember the experience you just had, you may not want to go out immediately again.  You can sometimes lose some memory of your previous one.

My advice -quit the Galantamine; it will become a crutch and finally, a handicap.

2 -Explore a variety of techniques; I like the WBTB early on...

3 -A big part of this initially, is that many of us get stuck for various, sometimes small, sometimes significant reasons; it can be very difficult to diagnose for others. You may need to explore finding inner permission, from yourself, from your higher self. You may just need more practice, more reps. You may be focussing on the wrong subtle aspects of the trance/exit process. Read more, study more, get reps, ask permission from high and low, inner and outer.

4 -This could just be me working on my empathic sensibilities trying to read you, but I get something both from your title and post that tells me you may be avoiding an issue that may be holding you back and is staring you in the face. Is there anything in this that strikes a chord?

I'll go even further than EV. Loose all mind and mood altaring substances including nicotine, alcohol and caffiene. Second all the problems you mentioned stem from one core skill which is focus. Focus is the most important skiill you can have in any kind of skill like this wether it be scrying, AP mediumistic skills whatever. Mantra work may be helpful and practice at least a couple times a day. Personally I don't practice skills at nighttime. I'm sleepy and my focus is bad. I do in the morning and sometimes afternoon maybe early evening if I'm still awake enough. You need to be able to do the skill on demand. It's very challenging, but to have what your looking for the work must be done.

Geesh I'm starting to sound like my teacher. LOL
