No, I think the seam where his finger meshes with the nose looks unnaturally sharp, and the shading is a bit off- could be real, but my experience tells me it is a computer altered photo.

But someone could try and do it themself

I've seen this before. It was definitely done in photoshop.

one of the giveaways to this is not only the unnatural length of his finger, but also the fact that it would have to bend in a way that fingers can't.
